RESOLUTION AUTHORIZING THE HONORABLE CITY MAYOR ROLEN C. PAULINO TO ENTER INTO,
EXECUTE, AND SIGN FOR AND ON BEHALF OF JAMES L. GORDON MEMORIAL HOSPITAL
(JLGMH) THE MEMORANDUM OF AGREEMENT (MOA) WITH THE LIFE AND HEALTH HMP, INC. AS
AN ACCREDITED HEALTH INSURANCE PROVIDER, IN A MANNER CONSISTENT WITH LAW.
WHEREAS, the Department of Health
recognizes the participation of private sectors for quality improvement in
health system;
WHEREAS, Life and Health HMP, Inc., a
corporation duly organized and existing under and by virtue of the Republic of
the Philippines, duly represented by its EVP for Operations, Catherine B.
Cerna; and referred to as the COMPANY;
WHEREAS,
James L. Gordon Memorial Hospital, a local government owned tertiary hospital existing under Philippine
laws and is represented by the Honorable Mayor, Rolen C. Paulino; and is
referred to as the HOSPITAL;
WHEREAS,
the Company has requested the Hospital to offer its medical facilities to
the Company for the benefit of the latter's employees, clients, members, and
the hospital has agreed to accept the said request;
WHEREAS,
all mutual accreditation, hospital and company's prestation, effectivity and
termination of this agreement are all indicated and enumerated in this
Memorandum of Agreement;
NOW,
THEREFORE, on motion of City Councilor Eduardo G. Guerrero, with the
unanimous accord of the Members of the Sangguniang Panlungsod present;

RESOLVED,
AS IT HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled,
to authorize, as it hereby authorizes the Honorable City Mayor Rolen C. Paulino
to enter into, execute, and sign for and on behalf of James L. Gordon Memorial
Hospital (JLGMH) the Memorandum of Agreement (MOA) with the Life and Health
HMP, Inc. as an Accredited Health Insurance Provider, in a manner consistent
with law.
Let a copy of the Memorandum of Agreement (MOA) be
made as integral part of this Resolution.
