RESOLUTION NO. 51

A RESOLUTION AUTHORIZING THE HONORABLE CITY MAYOR ROLEN C. PAULINO TO ENTER INTO, EXECUTE, AND SIGN ON BEHALF OF JAMES L. GORDON MEMORIAL HOSPITAL (JLGMH) FOR THE MEMORANDUM OF AGREEMENT (MOA) WITH THE CCS BIOMEDICAL TRADING CORPORATION FOR THE PREVENTIVE MAINTENANCE AND CALIBRATION OF MEDICAL EQUIPMENTS  IN A MANNER CONSISTENT WITH LAW

           

WHEREAS, the CCS Biomedical Trading Corporation, a company duly recognized and existing  and pertinent laws of the Republic of the Philippines, represented by its General Manager, GLENN V. CORONIA (Contractor).

WHEREAS, the James L. Gordon Memorial Hospital (JLGMH), a tertiary government health care facility managed by the City Government of Olongapo, existing pursuant to the laws of the Republic of the Philippines represented by the Honorable Mayor ROLEN C. PAULINO (Customer).

WHEREAS, the CCS Biomedical and training Corporation is engaged on maintenance services of medical equipment consisting of, but not limited to, general check-up, calibration and performance verification test and repair services. 

WHEREAS, the JLGMH requires the services of a qualified service provider for the preventive maintenance and calibration of its medical equipment on which it shall be performed at its site.

WHEREAS, the CONTRACTOR agrees to provide and CUSTOMER agrees to accept the terms and conditions as stipulated in the Agreement, maintenance service for the subject equipment.

            NOW, THEREFORE, on motion of City Councilor Eduardo G. Guerrero, with the unanimous accord of the Members of the Sangguniang Panlungsod present;

            RESOLVED, AS IT HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize as it hereby authorizes  the Honorable City Mayor Rolen C. Paulino to enter into, execute, and sign on behalf of the James L. Gordon Memorial Hospital (JLGMH) for the Memorandum of Agreement (MOA) with the CCS Biomedical Trading Corporation for the preventive maintenance and calibration of medical equipments in a manner consistent with law.

            Let a copy of the Memorandum of Agreement (MOA) be made as integral part of this Resolution.
