RESOLUTION AUTHORIZING THE HONORABLE CITY MAYOR ROLEN C. PAULINO TO ENTER INTO,
EXECUTE, AND SIGN FOR AND ON BEHALF OF JAMES L. GORDON MEMORIAL HOSPITAL
(JLGMH) THE MEMORANDUM OF AGREEMENT (MOA) WITH MEDICARD PHILIPPINES, INC., AS AN ACCREDITED HEALTH INSURANCE
PROVIDER IN A MANNER CONSISTENT WITH LAW.
WHEREAS, MEDICARD Philippines, Inc. a duly licensed managed health care company
providing out-patient, preventive and in-patient services to its members and is
represented by its Provider Relations Assistant Manager, Lulu C. Terencio, MD;
WHEREAS, James L. Gordon Memorial
Hospital (JLGMH) accommodates health insurance providers which cater to
patients who voluntarily avail of health insurance from private firms, either
independent of and represented by the Honorable Mayor, Rolen C. Paulino;
WHEREAS,
both parties shall accredit each other as partner institutions, hence, either
may use the name each other in its brochures, publications, and other
advertising and information materials;
WHEREAS,
the JLGMH and the MEDICARD shall exercise diligent coordination on all matters
pertaining to any member of MEDICARD being admitted at JLGMH and both parties
agree to establish mutually acceptable recording, billings and accounting
systems and procedures;
WHEREAS,
all mutual accreditation, hospital prestations, MEDICARD prestations and
effectivity and termination of this agreement are all incorporated and
enumerated in this Memorandum of Agreement.
NOW,
THEREFORE, on motion of City Councilor Eduardo G. Guerrero, with the
unanimous accord of the Members of the Sangguniang Panlungsod present;

RESOLVED,
AS IT HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled,
to authorize, as it hereby authorizes the Honorable City Mayor Rolen C. Paulino
to enter into, execute, and sign for and
on behalf of James L. Gordon Memorial Hospital
the Memorandum of Agreement (MOA) with MEDICARD Philippines, Inc. as an
accredited health insurance provider in a manner consistent with law.
Let a copy of the Memorandum of
Agreement (MOA) be made as integral part of this Resolution.
