RESOLUTION AUTHORIZING THE HONORABLE CITY MAYOR ROLEN C. PAULINO TO ENTER INTO,
EXECUTE, AND SIGN FOR AND ON BEHALF OF JAMES L. GORDON MEMORIAL HOSPITAL
(JLGMH), FOR THE RENEWAL OF THE AFFILIATION AGREEMENT WITH FORTUNE MEDICARE,
INC. (FORTUNECARE) IN A MANNER CONSISTENT WITH LAW
WHEREAS,
Fortune Medicare, Inc. (FORTUNECARE), a corporation duly organized and existing
under and by virtue of the laws of the Republic of the Philippines, represented
by its President and COO, Geronimo V.
Francisco.
WHEREAS,
James L. Gordon
Memorial Hospital (JLGMH), duly licensed hospital operated by the Local
Government of Olongapo City and existing under the laws of the Republic of the
Philippines, represented by the Honorable Mayor, Rolen C. Paulino.
WHEREAS, FORTUNECARE, in order to duly
provide the benefits and services of its healthcare program to its enrolled
members and dependents, requires the use and services of James L. Gordon Memorial
Hospital (JLGMH);
WHEREAS, James L. Gordon Memorial
Hospital (JLGMH), is fully capable of providing said services and facilities to
any authorized member/member-dependent of FORTUNECARE.
NOW,
THEREFORE, on motion of City Councilor Eduardo G. Guerrero, with the
unanimous accord of the Members of the Sangguniang Panlungsod present;
RESOLVED,
AS IT HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to
authorize as it hereby authorizes the
Honorable City Mayor Rolen C. Paulino to enter into, execute, and sign for and
on behalf of James L. Gordon Memorial Hospital
(JLGMH) for the renewal of the Affiliation
Agreement with the Fortune Medicare, Inc. (FORTUNECARE) in a manner consistent
with law.
